Gender dysphoria

Distress at the difference between your gender and how your body or the world treats it.

Gender dysphoria is distress arising from the difference between someone's gender and their body, or between their gender and how other people treat them. The first is usually called physical dysphoria and the second social dysphoria, and they are not always present together.

It is also a diagnosis, which is where most of the argument about the word lives. Being trans is not a disorder; the diagnosis exists because in most countries it is the gate to medical care, and that arrangement is criticised by trans people and clinicians alike.

This matters practically: you do not need dysphoria to be trans. Some trans people experience very little, some experience it intensely, and some describe gender euphoria as the clearer signal. Treating dysphoria as the test excludes people whose experience is simply not built that way.

What it is not is a question for a dating app. Whether someone experiences dysphoria, about what, and how much, is between them and people they have chosen to tell.

This entry explains the word. Anything about diagnosis or treatment belongs with a clinician or a trans health service, not with us.

Dysphoria is not the test for being trans

The diagnosis exists because it unlocks care, and that has quietly turned it into a requirement people believe they must demonstrate. Plenty of trans people have little or none.

Gender euphoria is increasingly described as the clearer signal: the feeling when something lands right, rather than the distress when it does not.

Saying it well

Say this

  • I get dysphoric about my voice.
  • Social dysphoria, mostly.
  • Not everyone who is trans has dysphoria.
  • That is not something I discuss on a first message.

Not this

  • So you hate your body
  • If you had dysphoria you would want surgery
  • No dysphoria means not really trans
  • Do you have dysphoria?
